I know that med students do research, but if someone's title is "research fellow" does that mean they graduated med school? I ask because I know anatomy fellows aren't always graduates but people taking a fifth year or taking a break before third or fourth year.

What about research fellows?
 
It means they are the recipient of a research fellowship- an award that allows them to continue their research, often at an institution separate from where they normally do research. It does not necessarily mean they have graduated medical school.

I was awarded a research fellowship at the University of Cambridge, UK for the summer after I graduated from undergrad. My title was "research fellow" while I was there.
