sending transcript and matching form

I live like 900miles away from my school, and my school has a transcript center, the registrars don't bother with transcripts. So... how do i get this transcript matching form turned in along with my transcript?
Has anyone requested a sealed transcript and then completed the matching form by him/herself? and did it work?

Is there a contact number for your transcript center? I just called that and the lady told me where to fax the matching form so they could include it with the transcript. It's not weird to require an additional form they will be familiar with the idea.
