so, i know that no one knows how this exam actually works, or maybe you guys have heard of stuff. Based on your own personal experience, if you can maybe shed some light.

1) out of the 12 cases, how many cases can you do poorly on and still do well.
2) how many cases have yo filled out all the 3dd's and how many cases just one?
3) for the PE can doing just focused and not listening to heart/lungs in all the pt be ok?
4) absolutely missing out a couple of dd/s for some cases
5) for the pertinent negatives, writing it in the HOPI and not writing it in the dd's, and not writing even a single pertinent negative as your supporting history finding. Have people done this and passed?

1. NOT sure, but based on previous experience shared by others, you can pass even with below average to poor performance up to 3 cases

2. I personally know couple of persons who just filled 2 DD's for all 12 cases and passed with high ICE and some other who filled 2 DD's for most of the cases and 1 DD for couple of cases and still passed.

3. Focused PE is all you need but not Lung/Heart for all cases...you wont get any credit for doing Lung/Heart PE for irrelevant cases. Example: No need to do Lung/Heart for MSK , Neuro and Abdomen cases most of the time unless and until SP gives some significant Hx and your DD demands for Lung/Heart. Official website actually recommends to do Relevant Hx and Focused PE.

4. I think you will be fine even if you miss couple of cases with irrelevant DD's.

5. Yes, people passed even with just mentioning enough +ve findings in supporting but adding relevant -ve things in supporting may increase ICE score. You have mentioned +ve and -ve things in HPI, so your HPI will be get good points.

Exam outcome is based on overall performance and again it is scaled and adjusted based on stringent SP's and Cases....You will be ok if overall performance is decent. All the best
 
I just took the exam last week, so I don't know how good my thoughts are on the test until I find out if I passed it, but overall, it wasn't bad. From what I was told going into the exam, the main problem I heard people ran into was time management. They either didn't finish their note or didn't finish the patient encounter. As long as you talk to the patient at the end and talk about what you're going to do for them and answer the question, I felt like it was fine. Also, I definitely feel like I missed some stuff in terms of question and exam practices, but overall, I was able to come up with 3 DDs for everything and a rationale for them all. I typed the Diagnostic tests and DDs first before filling in the HPI. Definitely kept running into the problem of not having enough space in the HPI, so I wish I practiced being more succinct, but other than that, I felt like it was not that bad overall. You just have to stay focused, be attentive and kind, and make sure to say everything you are doing to the patient.
