Setting up Observership

I am an applicant who graduated from an allopathic US medical school >5 years ago. Did an internship but left for another career. Looking to get back into medicine for anesthesia. I realize that my chances of matching are not high this year because of my long absence. I know its too late to set up an observership for an LOR this year, but what is the best way to set one up in case I don't match? If I don't match, I want to improve my chances next year. My former institution doesn't do observerships due to malpractice reasons.

It's hard to commit malpractice when you're not doing anything but standing there trying to stay out of the way. You might be a HIPAA risk, but not malpractice.
Go do anesthesia research for a year somewhere. During the course of the research, you might get patient contact, and you can observe some stuff on your down time.
Many places pick up research people for the summer or a year or two while people are putting together applications. They help with the studies, databases, etc. We have several.
