shirt and tie for orientation?

There's no reason why it shouldn't be casual. Most orientations are long, full-day activities....dress comfortably.... and you'll probably be the only one with a tie.
 
If in doubt, wouldn't it be better to be overdressed than under-dressed? You can always take the tie off 🙂
 
Shorts, T-shirt, and sandals... I'm sorry if you go to a stuck-up med school.

(If you're really worried about it, take a change of clothing in gym duffle that looks like you're on your way to work out after orientation.)
 
only consideration to add: if your department has pictures scheduled for tomorrow, then you probably will want to dress up. Otherwise, I say casual is fine; we started our orientation earlier this week and I've noticed the dress code has eased over the past couple of days...

chirurgeon, we already sent pictures, did everyone wear shirt and tie the 1st day for your orientation?
 
Unless your program is known to be very conservative, I'd go with what the non-medical world calls business casual. Pants and a collared shirt for guys - could be a polo or a button up, but no tie needed. Girls can get away with pants or skirt with a semi-nice top. I'd say dress like you are going to meet the parents of a new girlfriend or boyfriend for dinner - not so casual that it's shorts or jeans, but not so dressy it looks like you're trying to overly impress.
 
I wouldn't wear shorts and sandals as you might be expected to round with your new team, venture onto the floors, etc. You are after all now a representative of the hospital.

but as others have noted, "business casual" should be fine. Collared shirt, no tie and Docker's type pants for me and something similar for women. Ask your program coordinator if you are unsure.
