So I am so out of the circuit here, but I feel like I might have a chance at one of these out of the match spots that opens up. How do I find out about them? Should I call every program and ask them if they have any spots open outside the match for 2010?

I'm assuming this means you have done/are doing an internship, which is an obvious must. Honestly, there may be no better way to find out about an available out-of-the-match spot so I guess I'd say you should go for it. After all, you can't expect word of mouth and your own personal connections to inform you of every available spot, right? Squeaky wheel gets the grease and all that, right? Good luck!
 
I would say work through your PD and chair - do not cold call. Whomever has an open spot would want to hear from a colleague about an applicant, rather than an annoying stranger. They will be much more forthcoming about open spots in the wings with someone they know rather than someone they don't.

Hopefully you'll already be in good with your PD/chair (thus the emphasis on kissing ass if it doesn't make you barf).

Try to filter the list somehow (easier said than done, otherwise you wouldn't have posted). Region of interest, connections (per your own programs PD/Chair or the PD/Chair of away rotation you may have completed).

Having someone call on your behalf is probably the best advice, already provided. It acts more objectively on your behalf. Sure, the person speaking for you may be biased in your favor. But there is a reason for this and they will be able to better share these reasons.
