Signing a longer contract for fellowship

I wonder if anyone is aware of a precedent for being able to enter a fellowship with a longer contract in place - essentially saying that you will stay on as an attending for a specified period after completing you fellowship. I'm specifically interested as someone on a visa who will need to apply for a "waiver job" after residency - which means working in an underserved area for 3 years. I am trying to see if I can engineer a 2 year C&A fellowship thats primarily at an underserved site into a 3 year position that I can use to serve as my "waiver".
 
well technically you could extend your C&A fellowship for 3 years. but you can't get a J1 waiver as a resident, so you can't use time in residency as part of your 3 years, whether that is adult or child residency. it is only once you have finished your training you can apply for a J1 waiver. it is a sucky position to be in.
 
Doing certain agreements can only be done before or after the MATCH unless the fellowship is not part of the MATCH. To do so with the MATCH, I believe, would violate the programs participation in it. One of the reasons why the MATCH was made was because several programs were corruptly and unethically gouging applicants. "You want to go to our esteemed program where you will likely make an extra $50K a year? Well then, ahem, we won't pay you as a fellow. In fact, since, we will get a Medicaid reimbursement for your salary, that salary will go into my pocket. And, oh, by the way, I need someone to wash my car every week."

This type of crap did happen. That's why making arrangements outside the MATCH can get real sketchy and C&A is part of the MATCH unless you get into a program before the process starts. The MATCH puts rules to not negotiate anything outside of the rules because it's intent was to prevent applicants from being victimized from the type of stuff I mentioned above.
