signing matching form?

Okay so I was planning on having my dad go in to the dentist that is writing me a LOR to give him the LOR matching form (I'm up at school, he's at home and its like 4 hours away). My dentist is expecting my dad, so that's not a problem. I realized that I have to sign the LOR matching form along with my dentist. Do you think it would be a problem if my dad signed my name for me or what do you suggest? The dentist isn't very computer savvy so the electronic submission isn't an option but if I don't get it in tomorrow I won't be able to get it to him for at least another week, if not longer. What would you do?
 
anyone? 😕 sorry im trying to get it taken care of first thing tomorrow and im not sure what to do
