is this appropriate way to do MMSE

1) i ask them to repeat 3 words (apple,bat,cat)
2) ask them person,place,time
3) spell world backward
4) repeat the 3 words again



in First aid Pt note example, they listed in the Physical exam box a pt has "failed 3 item recall"

if a pt cant spell world backward, how would we write that in the note?
if a pt lists only 1/3 or 2/3 for person,place,time - how would we write that in the note?
