SNF MIPS quality measures

For those that round at SNF’s what quality measures are you tracking for MIPS compliance? Any good workarounds to be excluded from having to report?

Any of your EMR’s make this easy to do? I’ve been using practice fusion and it’s been ok at best.

154- fall risk assessment
155- Fall -plan of care
181 - elder maltreatment screen
282- dementia functional status
283- Dementia associated behaviors
293- Parkinson's
386- ALS

We use our charge capture or standalone EMR that is integrated with PCC/Matrix. No idea about workaround.
