Social security number in certification of identity form?

I'm trying to apply for step 3 now and I found the certification of identity form from the following link: http://www.fsmb.org/pdf/USMLEStep3_Certification_of_identity.pdf
It asks me to provide S.S./N. I. number, but as a foreign citizen on B1 visa, I don't have these numbers, what should I fill in?
In addition, it asks me to fill the "state licensing authority for which step 3 is being taken", should I just fill in the name of the state to which I'm going to send my application.

Are you aware that the deadline for the current application cycle for Step 3 is September 9? They must have all the necessary documents (if mailing, send them overnite courier and get a tracking number for proof of receipt) by that date or your application will be canceled. Your $730 or higher application fee will be forfeited; it will not transfer to the next cycle.

I hope you get things sorted out soon. You really need to process your security identity form. There are so many cases of identity theft nowadays and I bet you don't want to be a victim. Before you know it, somebody might have been using you identity here for something malicious. This is something that needs utmost importance and hurry up it's almost deadline.

It shouldn't take that long to process it if you all the requirements. Make a checklist to make sure that you have everything on hand before you get there. Goodluck!
