How exactly are specialists reimbursed or paid?

The reason I ask is because from what I understand (please correct me if this is wrong), the reason PCPs are paid so embarrassingly little is because insurances/Medicare reimburses these services so little and that Medicare largely sets these rate.. So does Medicare value specialist care more heavily and thus reimburses these services to a larger degree? Or is this not how specialists are paid? Are specialists pain in a fee-of-service like system?
 
In general the way things are currently set up is that "procedures" which include colonoscopies to punch biopsies pay out at a higher rate rather than a "simple" clinic visit to your PCP. That's why we have the health issues we have in this country. Preventive care is not deemed as important as procedures.
