One of these is not like the others. Hours and pay are lifestyle issues, while competitiveness is an entry issue (I assume that's the context in which you mean it). Just bust your ass and do well in med school -- it's only four years, compared to a lifetime of doing whatever specialty you want.

PM&R is not "great pay." When we say "great pay" in the context of medicine I'm thinking 300-800k. Any specialty can make 200k.

Sub 300k is great pay and not any specialty can earn 200+ with great or low hours.
