I think you're focused on the wrong thing. Applying with little to no clinical/non-clinical volunteering is not a good idea. If you get those hours up, you will be competitive at many good schools, possibly even some T-20s.
 
From what I have observed, MD-PhD at top schools have even higher stats than MD applicants, so I think 516 is unfortunately too low

Plus you might be asked why not a PhD. only since you have self reported little to no clinical experience . Do you have nonclinical volunteering to the unserved/underserved in your community?
 
like I Have ~120 hours volunteered at a hospital. ~200 shadowing. My non-clinical volunteer around ~400 at soup kitchens/food banks.
Well you didn’t indicate that in your original post. You have more than enough shadowing as long as you have some within primary care doc. Continue the hospital volunteering. You need 150+ hours at least.

I shadowed a Pediatric Oncologist, General Surgery, Sports Medicine, and Anethesiologist, does that count as primary care doc?
 
Those aren't primary care, sadly, except for maybe the sports-medicine doc. Double your hospital-volunteering hours, and you should have a solid application. Top-20 MD-PhD? That is likely going to be a reach for you, but top-20 MDs might bite if you are lucky.
 
My gpa 3.93, sgpa 3.90. MCAT 516 130/128/129/129

Research heavy, several publication but low impact and middle of the pack author. abstracts, and poster presentations.

Mostly do research, and not much clinical volunteering besides heavy shadowing doctors.

Should I apply T-20 with the low MCAT compared to the usual 518-523 MCAT average for most T-20 schools?
