Stand alone pictures or within context of question?

On the actual exam, are the images stand alone or within the context of the question? Both are present in UW but the NBMEs I've done don't seem to have stand alone.
But that I mean you wouldn't be able to get the question right if you didn't know what the image was.

If there is an image on the text, you would need to understand the image to answer the question correctly. The text may narrow your choices in some cases but you definitely need to know how to evaluate the image. On my test, all the images were accompanied by text.,
