Starting a Pain Practice...

I am currently a pain fellow. I am planning on starting a practice out west and looking for any and all advice I can get. I know its risky, and quickly understanding how much I don't know, but trying to do as much homework as I can.
Do you all have recommendations on things like EMR choice, building size, ASC vs in-office procedure suite, distributors, need for practice management group or not, etc.

I'd appreciate any input you have. Thank you in advance!!
