1. Do any states require a SEPARATE exam to gain state licensure outside of the NBPME board exams (steps 1-3). And what is the PM Lexis exam - just another name for NBPME step 3?

2. Do any states require full completion of residency before even applying for a state license? This seems counterintuitive since it seems many of the applicants for state licensure would be residents about to complete their residency requirements.

I took an oral exam during Residency to get licensed in AZ.
 
Most states require 1+yr of residency completion before a full license is granted. A fair amount of states have their own licensing academic exam, or at least a jurisprudence exam on the laws regarding DPM practice in their state. You an visit the individual state government websites to find out the DPM licensing laws and pre-reqs.
