Statement of purpose letterhead?

Hi,

This is a technical question but have not found the answer in similar threads.

I am applying online and instructed to copy + paste my personal statement from a word document onto the online application. Do I need to have the statement on some sort of a ( copied) letterhead, similarly to the appearance of a CV? Or would a plain document with no frills do?

Also, Should there be a title, and if so, should it be " Statement of Purpose" or Personal Statement"?

I'm in the midst of completing applications as well so not a reliable knowledge base, but I would tailor the title according to the individual program. If they ask for a Statement of Purpose, I would use "Statement of Purpose" as the title. If they ask for a Personal Statement, then I would use that for the title.

Also, I am including the title and my name beneath it just because I don't know if the application package gets printed off and looked at as a loose sheaf of paper, where pages may possibly get mixed up and shuffled around with other applications.
 
I'm not going to use a title. I don't think titling it "statement of purpose" or "personal statement" adds any additional info and it would count towards the overall number of words. I do think it's a good idea to put your name in the header so it's on all the pages.

Is someone able, perhaps, to demonstrate what the top of the page should actually look like?
Should the name be used in a header or on the same line as Statement of Purpose? Below it? Sorry, just a little tired today and could use a visual! ( obviously, not asking for any actual names; just a demo!)

Put your name in the header! Where page numbers sometimes go.
