A low score is a strike against you, no question. But I bet people will match this year with a score like yours.
Dr. Cuts' advice is good, of course: apply early and often, as they say. (For example, during one of your easy rotations, you can start to pull together your personal statement, CV, etc.)
But in addition, you need to put together your app in such a way that you make a good case for the fact that (1) there are many good things about you, and (2) even though you (all but) failed Step 1, you're a good bet to pass the radiology boards (which are apparently VERY hard). This is key, because program directors don't want to take a resident who's going to fail the boards, especially when there are so many high-scoring applicants out there. Boards pass rates is one of the few quantitative measures of a program.
What are the other things you can put on your app that will make you look good? Clinical honors help. And if you're serious about radiology, you'll go and get them. Research can help; you don't NECESSARILY need to take a year off for it, but you might.
Find a radiology advisor to strategize with. Working with him/her, figure out how you can sell yourself as a great candidate, who just happens to have a low score. You need better advice than you can get on SDN. 🙂
