Is having cousins at the state count as strong ties? Especially if I visit like almost every year? How would the med school even verify if I have cousins there and how often I went to visit. Couldn't some people just make up that they had their old grandfather live there for a long time then he passed away.

No.

The taxpayers of the state are subsidizing your tuition, so they want to know that you're going to stick around to practice.

Real ties are living in the state for a prolonged period of time, or having gone to college there.

So it probably makes no difference to say if you have cousins there or not. From my understanding you are saying that real ties are if you personally have been to the state for a period of time. Having family there is not considered having real ties then if you have not lived there for a period of time?

Having family there is NOT a tie.
