studying using USMLE World

Hi, I have six months left before my Step I exam. I have been extensively reviewing information using my class notes and review books, however I cannot break 50% correct on the USMLE World question bank. Can someone tell me what I am doing wrong? Why is it that doing well in my classes will not allow me to get good grades on this review question bank?

Also, I would like some schedule suggestions -

I cannot function on anything other than a 6am-10pm day, and I need to make sure to review every single class before the Step1, except Anatomy, Physiology, and Histology, which I know reasonably well. How am I going to spread out

pathology
microbiology
pharmacology
statistics/research topics such as type II error, sensitivity/specificity
genetics
biochemistry
embryology
neuroscience
clinical diagnosis
psychology/psychiatry

while studying for classes at school at the same time?

and my last question for tonight is:
I have difficulty memorizing the small details of my review books. I get tested on stuff like whether or not asthma triggers a cAMP or cGMP mediated pathway, whether or not a virus has a single or double stranded genome, etcetera. I learn the big ideas because it helps me to let knowledge stick, but this minutiae issue constantly comes back to beat me down. What needs to happen to stop this problem immediately? I am constantly on a wild goose chase, because when I memorize 10 facts, all the test creators do is test me on 10 other facts that I have never heard of.
