I'm not from the US but I have a question about the license first year residents hold. I know they still have part 3 of the USMLE to take so do they not have a license yet even though they're working a paid job practicing medicine.
Most US states (probably all) require a physician training license to start residency (i.e. to start working as a physician-in-training) and you need to pass step1 and step2 (ck/cs) to get a training license in virtually all US states. Step3 is needed to get an 'unrestricted' medical license.

To get an unrestricted medical license in any state, you must have a diploma, passed all of the USMLEs (including step 3), and have completed somewhere from 1-3 years of residency (depending on the state and where you went to school).
New graduates, with or without step 3, cannot be independently licensed. That said, they can either get a training license or practice under an institutional license in the context of a residency program. The exact specifics vary from state to state.
