My billing department has been having alot of trouble getting this covered.
First I tried using ICD G58.9 (Mononeuropathy Unspecified) with CPT 64450 (peripheral nerve block, other).
This was denied.
My biller suggested G57.9 (Unspecified mononeuropathy of the lower limb) which I did not think was as appropriate as the first code. We used the same CPT for peripheral nerve block again. That was also denied.
Now they are asking me if we can use the ICD 10 code for lumbar radiculopathy instead.
Any suggestions?
As a side note, the patient reports they have received 100% pain relief still ongoing 2 months out.

They are peripheral nerves, so 64450 should be most appropriate I think. There are many of them as well, so the other question is how many do you bill for? Typically there are 3 or 4 main branches of the superior cluneal nerves, for example.

I think it's considered one nerve. I just did an RFA on these buggers, but only billed for one nerve. It's not really a profitable procedure.
 
Are you guys using US or X-ray for the blocks? I’m assuming x-ray for RFN.
