hey folks, i know that san diego has a supplemental app. Does any other school have supplemental apps that I should worry about?

So this is what I have gathered so far, as far as supplemental information goes for programs...

UCSD - Form, $35 check, Photo
UCI - Photo, CV
Georgetown - Photo, CV, PS
UTSW - Form, Photo, CV
UTSA - Eye exam
U Fl - Eye exam, Photo - after interview request
Cleveland Clinic - Photo
Emory - Photo

Does anyone know of any other supplemental requirements?

Also, given that SF match has been messing up with the distributions this year (one coordinator told me that 24 applications were missing from a batch she was expecting) do you think it would be good to post where we have heard confirmation about an application being complete? Or should I just contact each program at some point to find out myself?

Are we expected to complete the supplemental apps automatically, or are we expected to wait to hear something from the program?

Of the programs I listed above, I think you are expected to submit information on your own except for Emory and the University of Florida, where I was contacted for extra information.
