One of the school I am applying to is asking me this question

"Have you ever been expelled, dismissed, suspended, placed on probation, or otherwise subject to any disciplinary sanction by any school, college, or university?"

I was placed on academic probation after my first semester in college due to having a GPA below a 2.0. I had to attend a mandatory meeting with a counselor. Is this something I should mention, or is the question asking me to only mention a case in which I was put on probation due to academic dishonesty like cheating or plagiarism or something like that?

The question asks if you've ever been on academic probation. You have, so don't make it worse on yourself and try to cover it up. Mention it, put a positive spin on it.

Yup, I agree with the positive spin part. You can talk about how you turned things around by fixing what you were doing wrong. All schools love an upward trend 😀.
