Do you know which schools require supplementary/special information in addition to the regular eras app?

I heard that vandy requires that we respond to a specific prompt in our personal statement.

UT southwestern requires that you answer 3 questions in lieu of a personal statement.

In lieu of is incorrect, I contacted UTSW,
You submit it as a personal statement, but address questions asked on their website

For me, I just retooled my personal statement pointing out where I answered the first two questions, and then just added another paragraph at the end addressing my ties to the area, family, friends, stuff like that.

For UTSW last year I submitted the personal statement as answers to questions, numbered
#1 blah
#2 blah
#3 blah
I got an interview there, so that format seemed to be fine!
