Thanks for posting. Looks like pretty sound advice. If I could get patients and surgeons to understand to stay on bupe for surgery it would solve a lot of headaches on our inpatient service. We have had a massive decrease in our post-op prescribing at our institution (~60% across the board over the last 5 years) but still run into some pockets where the practices haven't changed much.
