a friend is in IM [low tier program] just about to finish PGY1.
he wants to switch from IM to neurology. should he resign altogether from current IM program and apply to neuro from beginning next year? what are the chances? he is willing to get in any neuro position that he can get.

or he must find some one willing to switch position with him from neuro?

A lot of people who switch don't go through the match but rather directly through the programs with openings. There used to be a few websites where you could look for open spots, and for most of those you just apply directly to the programs. It's definitely worth even just emailing a bunch that he's interested in and asking if they accept transfers or have open spots
