When I put down on my app that I TAed biochemistry lab for a couple semesters, am I required to provide my professor's contact info?

If there's no way of getting this info, how can I prove to the schools that I was a teaching assistant for a year?
 
Put down the chair's info. The dept can still confirm your TAship and could also probably provide some commentary on your performance as well.
 
you never have to put info. if they have q's, they will ask you in the interview.

Is it just me, or are there a lot of these "how can I prove that my EC's are legit" questions this year? Nothing in the EC section will ever get verified unless some information comes to light that suggests you may not have been truthful about something. It's not like med schools are randomly calling your references, they haven't got time.

If you get tired after a couple holes, do not play a hole or two. Remember this is to be fun with your golf clubs for sale.
