TA including paragraph in the recommendation--how done?

I'm going to ask a professor for a recommendation, but it would also be very helpful to have the TA include a blurb in there since I am a lot closer to the TA. How is this normally done? Would the TA provide an additional printed statement that is sent along with the professor's recommendation, or, would I give the TA an entirely separate recommendation form?

I would have the TA give the professor a write-up of what he/she thinks of you. The prof can then use/include this when he writes his recommendation in his own words. Having LORs directly from TAs is frowned upon.
 
yeah this happens a lot. One of my letters was from a research PI and she's a department chair who i never saw for obvious reasons. A postdoc normally supervised me. The postdoc wrote most of it and collaborated with the PI, who signed off on it.

This is the best idea.

If the professor has done any sort of research, he should be pretty good at taking other people's opinions and presenting them as his own...
