Who usually provides this? For example, is it usually the current employer who provides tail coverage for your prior position, or is it the previous employer who provides tail coverage once you leave? Thanks!

Depends entirely on your contract.
Not in dermatology, but my first employer structured the contract so I was responsible for the tail. Big mistake.

After making that mistake, when I took my next job I was able to negotiate my next employer cover the tail. My contract is also structured so they will cover my current tail as well if/when I leave.

Yes it depends on your contract.

A nice compromise I have found is that if you choose to leave your job, both the physician and the employer contribute equally for tail coverage. If your employer fires you without cause, then the employer is on the hook for the tail insurance.

That is my current arrangement.

Thanks for your reply. I would like to negotiate for the employer to provide the coverage, but I'm not experienced with negotiation. Would it be effective to say something like "I'm looking at so and so offers that include tail coverage and signing bonus etc can you make your offer more competitive" ?

Equal contribution sounds reasonably fair

That is essentially what negotiation is all about: leveraging multiple offers to get the best possible one

I wouldn't lie. But if you do have multiple offers where the employer is offering tail coverage and a signing bonus, it never hurts to politely ask others in the negotiation to match or trump.
