3 questions:

1. I would like to eventually teach courses in med school (maybe basic science courses), do some clinical work, and perform public health research. Does this require an MD/PhD or will an MD suffice?

2. If just an MD, what additional training is needed?

3. How do the salaries of med school faculty compare with practicing physicians?

You never really need the MD/PhD, because everything a MD/PhD does can also be done by one who just has a MD. However, the MD/PhD is a specialized training pathway that is best for one who wants to devote most or all of their time to basic science or some other type of research. If you are interested in public health, there are a small number of programs that offer a PhD in epidemiology. If you want to teach basic science, you need to do research in basic science. I think some would say that researching basic science AND public health isn't really a feasible goal as either pathway requires all of you time. I'm not sure if this is the case, but perhaps you could keep this in mind.


2. If just an MD, what additional training is needed?

Perhaps an MPH or a post-residency fellowship that includes plenty of research in your area of interest.


3. How do the salaries of med school faculty compare with practicing physicians?

It depends on the specialty and the circumstances, as always. Generally, I hear that academic physicians make 1/2 to 1/3 their private practice counterparts. If anyone has any data on this I would love to see it.

an MD MPH might help. but then, i don't know, i have no interest in public health.
