Just wondering if this can be done, and if it will help my chances of matching to my ideal prelim. Unfortunately, what governs my prelim list is based mostly on LOCATION, and for obvious practical reasons, I want to either do prelim at the same city I will do neurology, or do prelim close to home/family.

Prelim programs seem to interview a lot of applicants (some very competitive ones applying for rads/derm/ophth), and the interview days tend to be short and combined with categorical medicine applicants. After these interviews, I never feel like I made a sufficient impression, simply because there are so many more applicants and only a handful get interviews with the PD or associate PD. Should I email these programs and make it clear I want to match there because the program is good (although secretly knowing it is location that matters most to me?)

I don't know that it's very "secretive" that location matters most... I think that's true for most people doing a prelim year. I suppose it wouldn't hurt to email them, but I don't think it would make much of an impression, since they know you'll likely want to end up at a program near where you match for categorical.
