Template for last year's application?

Hi I'm applying this up coming cycle. I know that the dental application doesn't open until June; however, I would like to know what information they will ask on the application by maybe taking a look at last cycle's template. Also, how many characters can you have for your personal statement?

4500 characters for personal statement.

Not sure If there's a template for the application. Just be ready to include pretty much your entire life and everything you've done the past few years..
