is it closer to the NBME practice exams or UW?

NBME wouldnt let me highlight or cross out anything and it didnt have a calculator

all of which world had..

how is the real thing?

It is identical to USMLE World

Just took the test yesterday.

It's nearly (if not entirely) identical to UWorld.
The only differences were on the heart sound windows, in which you're able to move the stethoscope over the four main listening areas. Also, once on my test they had a new window that popped up with 3 pictures instead of just one being included by the question. But beyond that, buttons are in the exact same place even.

My testing center also had physical calculators we could use if we didnt want to plug numbers into the computer.

You know those chart questions where there are columns and up/down arrows in for each thing? Are you able to cross them out on the real thing? You can't on UWorld
