the table from this site states that there were only 3 opthamology spots in the US available in recent years... how am i reading this wrong?

Ophthalmology match is done through the San Francisco Matching Program, therefore they do not list their positions with the NRMP.

I am curious what the three positions are for though...

ophtho is an early match, and via the SFmatch. Also, ophtho starts in pgy-2, not pgy-1. I'm guessing those 3 spots listed as pgy-1 are for prelim spots that are designated for matched ophtho residents.

This year, there was only one unfilled position through SFMatch. Number of unfilled slots has steadily declined over the past 10 years.
