Depends on what you are treating so I will help you though a hypothetical situation.
Assuming UTI, you typically do 8mg/kg of the trimethoprim component divided BID. For example, 17 kg child would need 136mg per day in divided doses, or 68 mg per dose.
Because there’s 40 mg of TMP in 5mL there’s 8mg in 1mL. To get to 68mg you would need 8.5mL to get 40 mg of TMP per dose, and you would do this twice per day.
