What do you want us to tell you? You're an adult, you need to make your own decisions. There are options for loan forgiveness in primary care
 
Could you transfer? Sure, if both institutions will allow it. Should you? We can't really answer that for you. As Psai said, there are options for loan forgiveness and loan repayment for entering primary care regardless of which option you choose.

I don't believe my dean has to "allow" anything? He has to give a letter of good standing if I request it. I just want to be in his good graces, so I'm wondering if my financial distress reasons are legitimate enough that he wont frown upon it?

Send him an email. I've only really heard of med school transfers in cases where someone has a spouse who attends a different school.
 
Academic administrators are necessarily divorced from the realities of personal finance. It's unlikely they'll approve the transfer.
