I tried SDN Search but came up empty-handed. 🙁

For those who have been through the application process, is the transcript matching form literally just a form provided to the schools to include with the transcript? As in, the school doesn't have to fill out any information? 😕
 
From the AADSAS instructions:

"This form contains the school’s name and a bar code that helps AADSAS properly match your official transcripts to your AADSAS application."

It doesn't sound like any additional info is needed...just helps AADSAS keep our stuff organized.

Yes, the transcript matching form is just a form provided to the schools to include with the transcript. When you enter the colleges you attended, for example a 4 year university and a community college, you have to print them out separately and give them to each school to submit to AADSAS. When you click "Print out matching forms", the name of the schools will already be on the form, since you already indicated that you attended these schools. From what I remember, all there is left to do is to print your name and sign it. You give this to your registrar's office and the only thing they have to do is to include it, they don't have to fill out anything.
