Where do I transcript request form? I can not find it on my PASS application. Anyone can help with this, thanks?

I went through this same thing, instead of selecting your degree type as doctor of dental medicine for your dental school, you will need to change it to doctorate of dental medicine. I’m not sure why it’s set up like that but after you change it the request form should show up.
