What should you consider when thinking about transfering to another FM residency and how difficult would it be?

I am in a large city program but have an interest in practicing in a more rural or suburban setting. The current program seems rather disorganized and am so frustrated with the conditions and concentration of generating income for the hospital rather than learning. However I am doing fairly well and feel that a program with an opening may have problems so by moving may go to a worse situation. So it may be better to stick this out.

If you can avoid it, don't do it. Also, there is no guarantee that you would get the full year's credit (that depends on the new program's PD and the board in Kentucky). If it is a parallel transfer (from FM to FM) you might. But why trade one set of headaches for a new set? And it just might prolong the lovely experience known as "residency training".

Doesnt sound to me like you have a very good reason for leaving. Stay put unless the situation is absolutely dire.
