Transitional Year/Prelim Year & Different Fields

So, I know there's no need to worry about this for a good long while, but out of curiosity:

Without going into the rationale behind asking, if you start off in a prelim/TY and end up deciding not to pursue neuro, pm&r, whatever, do you have a decent shot at landing back into IM or some other field that you typically start in as a PGY-I, as a PGY-II? You have to look for these spots independently of any placement/matching service I assume?

If you're in good standing with the program director, he/she would try to accomodate you within the program if possible or make some calls to other programs to help you out. At the minimum, you would want to complete the year so you can get a letter of recommendation and not have to repeat the year.
