Two questions about the application

Is it possible to list student club you were a part of on the application? I wanted to put it under extracurricular but it asks me for hours :/ .... I also wanted to list that I have an ASDA membership.. can I do that?

As for the question it asks in the background information "Has your education ever been interrupted or affected adversely for reasons other than deficiencies in conduct or academic performance?" can I talk about my injuries in this section?

For your first question, you should definitely put that you are a member of ASDA. I asked someone at admissions at one school and they said to list ASDA membership under "Extracurriculars." I hope this helps!
