Def Vandy! Aside from a preference for Chicago, seems like most things are equivalent EXCEPT the pass/fail clerkships. You really can't beat that. Vandy does have some graded clinical rotations, just so you know...just not the clerkship ones. Also Nashville is an amazing city that I'm sure you'll enjoy. Much better weather too.
 
In terms of P/F curriculum, I would go Vandy. However, if your friend is planning an MBA, I would go NU as Kellogg is hard to beat.

Having lived for a very, (very) brief period of time in both places, I would personally probably pick Northwestern solely due to location as I love the Near North Side MUCH more than Nashville. But if your friend has no preference for location (or love both equally) and is unlikely to want to pursue a dual degree, I agree with the other comments that Vandy seems like the better choice by program alone due to P/F, research time. Neither class size seems like a concern to me.
