i've searched about this topic on the forum already but doesn't seem like anyone really answered the question....so i'll re-post it. does anyone know if you can turn in the personal history form late? b/c i was out of town for several weeks and now i'm really late.

yes, it should be okay to turn it in after 10 days--i did. others posted that they called the ucla office, and they said it was okay. here's one of the threads:
