Hey guys a few questions (Again):
1. when they say: "PLEASE DESCRIBE THE DETAILS OF ANY SIGNIFICANT COMMUNITY SERVICE ACTIVITIES"
are they expecting a resume styled format simply about my tasks or do they also want to hear what I learned/insight I gained during the experience?


2. what if for some community services are student organized, do I give them the cell phone number of the president or what? but then, the president is a different one than when I was volunteering... what should I do?
2a. similarly, what if the supervisor for my former job changed, do I still give them the number?


3. for questions like:
".Do you come from a region/community where there is a shortage of medical and dental care, or where most of the care is provided by city, county, state, or federal clinics and hospitals? (e.g. rural community, inner-city community)".


Can I simply answer "No" or should I type a full sentence like "No, I do not come from such a region/community."

I did both. Like I said... I was responsible for doing this... and this helped with my social skills dealing with people, etc. But I think they want to hear more about activities/duties.

Don't give a phone number? Or give the phone number of the main organization where you were volunteering. Give whatever, don't think its that important.

Yes. I am sure your workplace regardless of supervisors has records of you working there.
