If you have 800 characters to explain where you see yourself in ten years, would you give a broad answer or would you try to be very specific.

By the way, I dislike this type of question. It is just too hypothetical.
 
This reminds me of a story I heard once:

So, this resident was interviewing for a faculty spot in the department of surgery at a top 10 program. The interviewer asked her, "Where do you see yourself in 10 years." She responded, "In your chair."

She was hired.

I'm not sure I'd be that specific, but I would be as specific as you can (e.g. surgery vs medicine; rural vs urban vs suburban; academic vs. community; hospital vs. outpatient), without feeling like you need to know exactly where you want to be in ten years.

Anka's right about being as specific as you can. It will also help if you say why you want to be there. For instance, if you want to practice in an urban setting, you might explain how you want to stay abreast of a wide variety of conditions and treat patients from diverse backgrounds. I know you don't have a lot of room in this essay, so you might have to just pick one of the many things you'd like to do, but the specifics will make your answer less "hypothetical."
