got a question-when you submitted your secondary application for UCLA, did you have to include a transcript matching form to your UCLA secondary application or did you submit your transcripts w/out a form?

What I did last year was to pick up all the transcripts in person. Then, send them along with the secondary application w/o the transcript matching form.

UCLA Requires TWO transcripts, one to ADSAS with a matching form and another copy of an official transcript sent directly to school or hand delivered to their office without any matching forms.

In my case, I had submitted a second copy even before I submitted my secondary. So do it asap.
