UCSF secondary: Form A problem!!

I have a trouble in completing Form A of the UCSF secondary.

For bio, My undergrad school (UC Irvine) offered lecture and lab separately. So I took 3 bio lectures + 3 bio labs....total 6 classes for bio prerequisites. But on the secondary form, there are only four spaces are given which means I can't type in my lab classes grades and course numbers..... I need more spaces to fill out my bio pre-req classes....How did you guys fill out the form A?
 
I just put two of the bio classes I took, as that there aren't particulars for the requirement, and then put 2 labs. They are just making sure that you will have the requirement filled before matriculation, I had taken a few other bio classes but I just put two. Also I made sure to put two of the classes I got A's in so just pick the two you did the best in. They obviously have access to your other grades so it isn't a big deal which two you choose.
 
hold up. that doesnt answer OP's question. he saying he got 3class+3lab but there are only 4 spaces. so how does that fit in the blanks?

Sure it does. The form wants you to list 2 biology classes + labs that make up the pre-reqs (xx amount of quarter units). So you get 4 spots. Pick any 2 bio classes + labs that completes their requirement.

it might be best to call the school, since they are the ones reviewing it
 
I contacted UCSF regarding this issue. They said to indicate just the course number

i.e.,

if your official transcript says BIO200 (BIOL200) --whatever the abbreviation is -- just indicate "200" since it already has the department name on the left hand side. if the lecture and lab were separate course, then 200/250 or something like that. it will fit that way. I had to do the same for organic chemistry and physics.
