from what I can tell from the e-mail that I got from their office, I think that the blue form is all we have to fill out to complete our files. in fact, i can post the text of this e-mail here now, make of it what you will. it says nothing of any other forms besides the blue form....
Dear Applicant:
The College of Medicine (COM) Office of Admissions has received your application from AMCAS. You will be receiving via U.S. Postal Service correspondence from the University of Florida Office of Admissions, not the COM office . Along with a cover letter the package will include a University of Florida Application for Admission to the College of Medicine, the blue form. Follow all instructions and return the form to the address provided.
The blue UF admissions application is not indication that a decision has been made by UF COM Office of Admissions. Although your AMCAS application has been received by this office a decision will not be made on your application until the UF admission application has been completed, returned and is on file in that office. Once your blue form is on file in the UF Admissions Office and a decision regarding your status in the College of Medicine Office of Admissions has been made you will be notified via e-mail.
If you have submitted the blue form and have not received notification from the COM Office of Admissions, one month after its submission you should verify your status with the UF Office of Admissions, 352-392-1365 Office of Graduate and Professional Programs.
